1. Observe the real work

Start with the people doing the job. Capture the normal path, the exception path, the workaround, and the information that arrives too late.

2. Map decisions and handoffs

Separate actions from decisions. Identify who owns each judgment, what evidence they use, and what changes when information is incomplete.

3. Bound the product

Write down what the system may assist, what it must not imply, and where a person must remain accountable.

4. Prototype the smallest useful loop

Build enough to test one uncertain assumption. A working prototype is evidence that something can run, not evidence that people need it or that it is safe.

5. Test and learn

Measure whether the workflow becomes clearer or more reliable. Record what remains unproven and choose the next test based on the largest responsible uncertainty.
